Injectables

Injectables in Sugar Land, Katy, and Houston, Texas

When we talk about injectables we typically refer to two types of substances: neuromodulators (Botox), fillers (hyaluronic acid such as Juvederm) or biostimulators (Sculptra or Radiesse).

It is important to understand how these 3 products work. First, let’s talk about neuromodulators or neurotoxins. There are 4 FDA-approved neuromodulators in the United States: Botox, Dysport, Xeomin and Jeuveau. All of these help relax the muscles of facial expression in order to smoothen dynanamic lines (those that appear when we express). Neuromodulators are typically used on the third upper portion of the face (forehead, midbrow and crow’s feet) to reduce lines that usually form from squinting, smiling, raising the eyebrows, or frowning.

Second, let’s go over fillers, what they are and what they are used for. Fillers are gel-like substances composed of hyaluronic acid that come in different formulations depending on the viscosity and cohesivity of the product. Fillers can be injected superficially in the upper dermis, deep dermis or on top of bone depending on the need and type of product.

Importantly, we favor the use of fillers that are temporary over those that are permanent (silicone or Bellafil) given safety concerns.

Here are some of the reasons for using injectable fillers:

  • Smooth out lines around the mouth and nose, also known as marionette lines or smile lines.
  • Remove vertical lip lines.
  • Enhance or plump the lips.
  • Smooth over a crease in the chin.
  • Improve the symmetry of the face.
  • Improve indented surgical or acne scars
  • Improve dark circles and grooves under the eye

Today, fillers come in various FDA-approved formulations. To ensure your safety, the injectables you use should always be FDA-approved and only be obtained through a licensed medical professional, such as a board-certified plastic surgeon or dermatologist. Examples of FDA-approved hyaluronic acid fillers include Juvederm Ultra, Volbella, Vollure, Volluma, Restylane, Restylane Lyft, Restylane Silk, Revanesse Versa, Belotero, and the new RHA Teoxane family of fillers (1,2,3 and 4).

Lastly, biostimulators such as Radiesse (calcium hydroxyapatite) or Sculptra (poly-L-lactic acid) are collagen fertilizers injected in the deep dermis or on top of bone to stimulate collagen production. The full results take many weeks to show and multiple sessions (typically 3) spaced out a month a part are recommended. These products should not be used around the eyes or mouth, but they can be used in a diluted way to correct dimples on the buttocks (cellulite), and skin laxity on the neck, abdomen, arms and legs.

FAQ

How much times does a Botox/Dysport/Jeuveau treatment take and will it hurt?

The injection process takes 10 minutes. Being injected with the neuromodulators feels like a slight pinch. To ease any discomfort, the practitioner can apply ice to numb the skin momentarily.

When do I see results from a Botox treatment and how long do they usually last?

Visible results will be noted 7-10 days after you receive the treatment. The treatment lasts about 3-4 months for frown lines considered moderate to severe.

Who is a good candidate for a Botox/Jeuveau Injectable?

Anyone between age 25-70 years or age who is not pregnant and that does not have certain conditions of the neuromuscular unit such as Eaton Lambert or myasthenia gravis.

What is hyaluronic acid (HA) used in some injectable dermal fillers?

Hyaluronic acid or HA is a complex sugar that occurs naturally in the skin. The substance keeps the skin hydrated and plumped, and, when used in fillers, is gel-like and soft. Most HA filler products are infused with lidocaine to minimize any discomfort during and after treatment. HA fillers last about 6 to 12 months, on average.

What are calcium hydroxylapatite (CaHA) fillers/biostimulator?

Calcium hydroxylapatite is a naturally occurring substance that occurs in the bone. When it is used as a filler, the substance’s calcium particles are suspended in a smooth gel. Usually, this filler is thicker than HA fillers, and lasts longer too – about 1 year. CaHA fillers stimulate the natural production of collagen and are normally used for deeper wrinkles and lines. A brand example of an FDA-approved CaHA filler is Radiesse®. Multiple sessions are recommended to achieve the best result.

How does poly-L-lactic acid work?

Poly-L-lactic acid can be used inside and outside the body. Therefore, the biodegradable synthetic substance is biocompatible. Also used in the making of dissolvable stitches, poly-L-lactic acid is considered a collagen stimulator and helps rebuild collagen. Sculptra is used as a volumizer of deeper wrinkles and the results last a long time – as long as 2 years. Multiple sessions are recommended to achieve the best result.

What are PMMA injectable fillers?

Polymethylmethacrylate or PMMA is a biocompatible synthetic filler. It has been used in medicine for around 100 years. When used in a dermal filler, PMMA is made into a collagen-infused tiny ball, or microsphere. It stays beneath the skin to provide structure and support. An example of an FDA-approved PMMA brand is Bellafill®. Given that this product contains bovine collagen, an allergy skin test is recommended prior to its use.

Are there any injectable filler treatments that require surgery?

Yes. Autologous fat injections, known as facial fat grafting, do require surgery. However, the results can last several years. The fat of the patient is harvested from another area of the body, typically by means of liposuction. That is why the procedure is referred to as “autologous” – which means from the same person. After the fat is extracted, it is refined and administered by way of injection to the facial area. Autologous fat injections restore volume and smoothness to areas, such as the lower eyelids, forehead, and cheeks.

How do I determine what kind of injectable I need?

A cosmetic doctor who is trained in administering injectables can help you decide what formula works the best for your skin tone and texture. Don’t make a decision without talking to a trained medical professional.
